    Outpatient Antibiotic Stewardship

TrackingCore Element Three: Tracking and Reporting (Audit and Feedback)

    1. Resources in development include : analytic reports, database for benchmarking by infection for providers individually or as a group (EMR Reports, NHSN, chart review), oral antibiotic Medicare part D regional prescribing heat maps, and site specific Medicare part D antibiotic prescribing reports based on provider NPI.
    2. Reports of complications of antibiotic prescribing- (C-diff, drug interactions, adverse drug events, resistance) C-Difficile and associated potential adverse drug event report from Medicare part D data.
    3. Self-Evaluation Audit Tools (Survey Monkey with four Core Elements and check box)

  1. Antibiotic Tracking Form

An antibiotic tracking form was developed to provide an assessment of oral antibiotic prescribing patterns to meet the CDC stewardship core element of tracking and reporting.  The form includes  annual (or quarterly) prescription level data entry (Medicare Part D or electronic health record data) and concomitant summaries and graphs are automatically generated. The tracking form can be used for committee level presentations (antibiotic stewardship and quality) , longitudinal assessments of antibiotic prescribing patterns, presentation of state level antibiotic prescribing patterns,  pre and post antibiotic stewardship implementation evaluation or as individual provider level  benchmarking.   The tracking form supports a variety of analyses including: across practice groups, within a practice by provider, infection specific prescribing analyses and practice type comparisons (primary care vs. specialty vs. urgent care vs. emergency).
