13 Long Stay Quality Measures

    1. Webinar - Percent of residents with one or more falls with major injury
      This measure reports the % of long stay residents who have experienced one or more falls with major injury reported in the target or look-back period

      This measure reports the % of long stay residents who have a urinary tract infection

    3. Webinar - Percent of Residents who self-report moderate to severe pain
      This measure captures the % of long stay residents who report either almost constant or frequesnt moderate to severe pain in the last 5 days, and horrible pain in the past 5 days

    4. Webinar – Percent of High Risk Residents with Pressure Ulcers
      This measure captures the % of long stay high risk residents with Stage II-IV pressure ulcers

    5. Webinar - Percent of Low risk residents who lose control of their bowel or bladder
      This measure reports the the % of long stay residents who frequently lose control of their bowel or bladder

      This measure reports the % of residents who have an indwelling catheter in the last 7 days

      This measure reports the percent of long stay residents who are physically restrained on a daily basis

      This measure reports the % of long stay residents whose need for help with late-loss Activities of Daily Living (ADL) has increased when compared to the  prior assessment

      This measure captures the % of long stay residents who had a weight loss of 5% or more in the last month or 10% or more in the last two quarters who were not on a physician prescribed weight loss regimen noted in an MDS assessment during the selected quarter

      This measure reports the % of long stay residents who have had symptoms of depression during the 2 week period preceding the MDS 3.0 target date

      This measure reports the % of long stay residents who are receiving antipsychotic drugs in the target period
