Improving Cardiac Health
AQIN offers technical assistance and a range of evidence-based resources to help home health agency staff improve the cardiac health of the patients they care for. We will also convene Learning and Action Networks (LANs) to give providers, community organizations and patients the opportunity to share, learn and make a difference.

Improving Care Coordination and Quality
Better coordination of patient transfers among care sites and the community could save money and improve the quality of care. This requires strong partnerships between long-term post-acute care settings like home health care and other providers such as hospitals, primary care physicians, and outpatient clinics.
